Lancashire County Council together with Health Commissioners invite suitably experienced organisations to provide a day time support service within 'Mere Brook' at Brookside centre in Ormskirk, West Lancashire.
Brookside Well Being resource centre is a joint integrated health and well-being centre which provides extra care housing and a range of support and services to local people living in West Lancashire.
The support services available at Brookside have been designed and developed to be innovative, cutting edge and inclusive and it is essential that this philosophy is continued and nurtured.
Brookside uses a collaborative partnership approach whereby organisations work together and provide seamless and accessible services from a community base.
The service is jointly funded by Lancashire County Council and NHS Central Lancashire/Clinical Commissioning Group West Lancashire.
Through the tendering process we would expect to have 1 host provider who would co ordinate and sub contract on a short to medium term basis activities and groups from other partner organisations to enable and provide day time supports that are truly person centred, vibrant and provide an innovative offer to people living with Dementia in West Lancashire.
We are seeking Expressions of Interest from single providers; then if you are shortlisted, at the Invitation to Tender stage there will be an opportunity for you to identify providers that you would sub contract parts of the service to.

Interested providers are invited to an information day on Tuesday 9 October at 1pm to be held at East Cliff County Offices, Preston.

Due to the venue size we are limited to only offering two places per organisation initially. Please ensure you book any places by Monday 8 October 2012.
Providers wishing to express an interest are required to complete an electronic pre qualification questionnaire via the e-tendering website.
Providers who are already registered on the e-tendering website will be able to access the questionnaire by logging in to the website. Providers who are not currently registered on the e-tendering website will need to go to the above web address and register as a new user.
Questionnaires must be completed and returned electronically via the e-tendering website by 10am on Friday 26 October 2012.
