News: Tender Opportunity Briefing Event - Wellbeing Worker Service for Vulnerable Adults
Lancashire County Council (LCC) as the commissioner is seeking to procure a Wellbeing Worker Service.
The service being procured is part of the plans for the Integration of wellbeing services and aims to support vulnerable adults, particularly those at risk of a health or social care crisis, to address the issues and underlying causes that are affecting their ability to be healthy. It is based on the principle of improving the wellbeing and resilience of vulnerable people, making use of the local community assets, which in turn will prevent, reduce or delay the need for more intensive and expensive health and social care interventions in the future.
The objectives of the service being procured are:
1. To work with partners and referring agencies raising awareness to create an effective seamless pathway to and from the service
2. To provide tailored support for citizens to improve their health and wellbeing
3. To connect citizens with other health and wellbeing services, and community assets
4. To proactively identify and engage with the citizens who would benefit from the service using targeted outreach and engagement with priority groups, communities of interest and in areas of high deprivation
5. To develop, implement and manage a volunteer function.
The service will be a targeted service rather than universal and will be delivered for the benefit of all citizens aged 18 years plus who live within the 12 Lancashire districts that meet the specified criteria.
(Please note the service does not include Blackpool or Blackburn with Darwen).
The service will undertake holistic wellbeing assessment using a standardised tool, to ascertain assets, needs, priorities, goals and support in partnership with the individual. Providing tailored support for the service user over a period of weeks, this will include signposting or supporting access to other agencies or services dependant on the needs of the individual.
It is likely that the tender will be published early next week, subsequent to being advertised on the Authority's Corporate website and full tender documents will be available via the Authority's Oracle (iSupplier) e-tendering system.
